via credit card. You will receive a user name & password with your immediate conformation, as well as on a receipt automatically forwarded to your email address. You will be presented with a choice of two files, both being absolutely identical except in file type. The difference being, one is in PDF format (Adobe Portable Document File) that you can immediately view. The other is a compressed into ZIP file that contains the same PDF but in a smaller file size for faster downloading and makes it easier to download.
Download either file to your computer.
If you download the PDF file version, it is recommended that you first download / save it to your computer if you experience any issues with browser security. Simply close your browser, then navigate to your newly saved file and open it with the latest version of Adobe Acrobat Reader freely available from www.adobe.com.
If you download the compressed ZIP file version, save it to your computer. De-compress it using windows or download WINZIP from www.winzip.com (or any zip decompression). Once de-compressed, you'll be left with the original PDF which you can then open with Adobe Acrobat Reader.
You can then view on your computer or print it to any
printer of your choice.

A formatted version on
8.5x11 inch paper, soft cover mechanically bound is available to those who generally can't print a copy themselves. It is
printed on a quality laser printer that is shipped to you shortly after receiving
confirmation of your order. Shipping is $4.75 via Priority Mail USPS. This is is 15% discount off the standard rate.
It will be shipped by Priority Mail - U.S. Postal Service to you in the US ONLY. Additionally, you are still given access to the download page once your order is complete.
Please note that the price of this has remained the same despite increases in postal fees, increased website fees, and increased credit card processing fees,increased paper and toner costs. In order to maintain the same price due to the forementioned, the hardcopy will only include the state indicated with your order. To include all unnecessary states on paper is also a waste of killing of even more trees. However, you are still free to download the entire US state listing should you be interested in additional state guidelines. In this way I can maintain the same price without passing on the ever increasing expenses of postage of Priority Mail, paper, laser toner, and other fees already mentioned.

TO PLACE AN ORDER :

which you can successfully access a password protected page after you receive your

Please note that in order to protect your card
from theft, address verification must match the address
assigned to your credit card (not a present address if different).
3. Upon successful authorization - obtaining the access information
Ordering by AuthorizeNet: After your authorization is approved, you will receive a confirmation
receipt page that is a copy of your transaction that you should print &
save. On this confirmation page will also be your Username &
Password, located in the footer of your receipt. Armed with
your new Username & Password you can gain access to the secure web page by clicking on
the "Download" button on the ORDER page. Enter the correct Username and Password to get in.
Download both files if you choose.
PAYPAL option:
Existing PAYPAL customers. Upon successful authorization you will be sent to a page providing your access information to use.
Non-PayPal customers who do NOT log into PayPal.
There is no automatic forwarding for customers who have not logged into an existing PayPal account. After successful authorization is approved, you MUST look for a link to click to take you back to the page providing your access information. If you do not, you will not receive this information.
If you ordered a hardcopy
version, I will receive an e-mail notification
of your purchase request. Upon opening your request, I will get
it in the mail within approximately 2 - 3 days after it is shipped. Shipment will be via United States Postal Service.
Please expect delays for weekend closures or holidays. If there is a
problem, you will be notified.
